Need to report a campus concern? There's a form for that!
Several options are available for students, faculty, and staff to make reports to the university. The Campus Climate & Incident Reporting form serves as an additional resource for those who may be unsure where to best file a grievance, express concerns about campus climate and safety issues, or who may think their concern "doesn't fit" under another umbrella. These reports will be reviewed by RISE, and routed to other departments, such as a Title IX Investigator, Student Conduct, or the Dean of Students Office when requested or appropriate. This form can be filled out anonymously if you choose. You can use this report for incidences pertaining to yourself/your group (Student Org), a friend you are concerned about, or an incident you witnessed but may not know either party. Please note- should you choose not to provide identifying information, RISE and University staff will not be able to follow up with you about your report.

You can use this form to report incidences such as:
  • Bullying
  • Campus Climate
     
  • Domestic Violence
  • Hate Crimes
  • Hate Speech
  • Hazing
  • Micro-Aggression
  • Physical Assault (non-intimate relationship)
  • Sexual Assault
  • Sexual Harassment (including speech, actions, etc.)
  • Sexual Voyeurism
  • Stalking
  • Other

If you would rather speak with someone face to face, the RISE staff can take reports in person Monday through Friday, 8:30 am to 4:30 pm. RISE is located in the Student Wellness Center, Suite 204, directly across from the Student Counseling Center. Please be advised that under Title IX, Staff in the RISE office are "Jane/John Doe" reporters- meaning, RISE would be required to make a report if warranted (Title IX violations, Clery crimes), but can do so using pseudonyms and other non-identifying information.
